Full Consultation

We focus in on exactly how to help yourself or a loved one move forward with practical solutions.  We help you understand what funding someone is eligible for, how to navigate government benefits like Medicare and Medicaid, deal with the changes that occur when someone’s health changes, and prioritize what to do next — so you don’t head down the wrong path and create more work for yourself.   

Family Assessment Meeting 

We facilitate meetings to get everyone on the same page regarding XYZ.  We make sure you understand what you or a loved one is eligible for, what the process looks like to get services and who can help if things get stuck.  You can take notes or we can create a plan of care based on your best options and the most efficient ways to put them in place.  

Private Case Management

If you want someone to handle the whole situation — assessment, plan of care, setting up services, support at medical appointments — we can do that. Having one point of contact works well when the situation is complicated, families are out of state or relationships are rocky. We provide several levels of case management depending on your needs.

Supportive Counseling

Our counseling is unique in that we have an expertise in aging and disability. We let you be seen and heard so you can move forward in your specific situation. 

Professional Personal Advocacy

We advocate for you during those times when you need someone to work on your behalf.  We are highly skilled at leveraging your rights and options, then following through for resolution.

Medicaid HCBS Applications

We complete Medicaid Home & Community-Based Services (HCBS) applications within the Denver metro region.  This includes submitting the proper documents, following up on the application, advocacy during the functional assessment and setting up services.  Note: this does not include finding a bed in assisted living or skilled nursing facility.
