Thinking about your healthcare decisions in advance can limit unexpected surprises for you and your loved ones. This free workshop will provide you with an opportunity to complete your Medical Durable Power of Attorney and Living Will forms with facilitation provided by Jill Eelkema, a masters-level social worker and Advance Care Planning-Certified volunteer. Once registered, you will need to bring:
- your ID (so forms can be notarized)
- the name, address, phone number and email addresses of at least three people whom you trust to make healthcare decisions if you can’t
- any previous Medical Power of Attorney paperwork you’ve filled out before (if you want to reference them)
